At Isuroon, we believe that safe and stable housing is the foundation for a healthy, empowered life. Our Housing Stabilization Services (HSS) are designed to support individuals with disabilities, mental health conditions, or other challenges that place them at risk of housing instability or homelessness.
As a Minnesota Department of Human Services (DHS) approved provider, we work alongside individuals, families, and case managers to deliver personalized support that helps people find and maintain housing that meets their needs.
How It Works
- Initial Consultation: Schedule a session with one of our certified housing counselors.
- Assessment: We’ll review your financial profile, credit, and housing goals.
- Education Plan: You’ll receive customized guidance or be enrolled in our homebuyer education workshop.
- Ongoing Support: Even after you purchase a home, we’re here for your questions and continued success.
What is Housing Stabilization Services (HSS)?
HSS is a Medical Assistance (MA) benefit aimed at helping people with disabilities and complex needs achieve housing stability through three core supports:

Housing Consultation
For individuals not currently receiving case management services, we help develop a person-centered housing plan, coordinate resources, and establish eligibility.

Housing Transition Services
We support individuals as they search for housing, complete rental applications, navigate housing barriers (such as poor rental history or lack of credit), and secure a lease.

Housing Sustaining Services
After housing is secured, we continue to provide support—helping clients maintain housing, build daily living skills, communicate with landlords, and connect with community resources.
